    Overview

    A multiple myeloma is a rare form of blood cancer that affects the plasma cells of the immune system. These plasma cells, also known as B-cells, produce immunoglobulins, or antibodies, that help the body defend against infection.

    Abnormal cells in the plasma become multiplied and mutated, creating a range of medical issues. Multiple myeloma can have serious effects on your bones, kidneys, and red and white blood cells, as well as your platelet count. Unfortunately, there is no cure, but treatment options can be used to slow the progression and manage symptoms.

    Plasma cells in people with multiple myeloma crowd out the normal blood cells in their bones, which leads to lytic lesions. Furthermore, these cells release chemicals that cause other cells to eat away at the bones, resulting in further damage.

    Types

    In people with multiple myeloma, the cancerous plasma cells produce abnormal proteins. These proteins can damage your bones, organs, and tissues. The disease can spread to other parts of the body.

    With multiple myeloma, the cancerous plasma cells divide and grow uncontrollably within the bone marrow. This causes the production of abnormal proteins, which can lead to damage to bones, organs, and tissues.

    Listed are some main subtypes of multiple myeloma.

    • Hyperdiploid: Approximately 45% of multiple myeloma cases are caused by an abnormal type of myeloma cell that has an increased number of chromosomes compared to normal cells. And this type of myeloma tends to be less aggressive.
    • Hypodiploid or non-hyperdiploid: Approximately 40% of individuals with myeloma have a more aggressive form of the disease, which is characterized by fewer chromosomes than normal myeloma cells.

    Different types of multiple myeloma exist, and certain precancerous conditions may cause the development of this condition. Those might include the following:

    • Light chain myeloma: Myeloma usually involves the production of immunoglobulins, but in some cases, only an incomplete form of these antibodies, known as light chain myeloma, is present. This occurs in 20% of myeloma patients and can lead to kidney damage.
    • Solitary plasmacytoma: Plasmacytoma is a tumor caused by cancerous plasma cells growing out of control; solitary plasmacytoma is when there is one tumor, while multiple myeloma is when there are multiple tumors.
    • Non-secretory Myeloma: People with non-secretory myeloma may not produce enough M proteins or light chains for tests to detect. A bone marrow biopsy is often used to diagnose this type of myeloma.
    • Extramedullary plasmacytoma: Extramedullary plasmacytomas are tumors in the body’s soft tissues, often occurring in the throat, sinuses, nose, and larynx. Treatment options include radiation therapy, surgery, or both, and may lead to multiple myeloma in up to 30% of cases.
    • IgD Myeloma: 1% to 2% of people with myeloma have this rare type, primarily affecting men under 60; signs and symptoms are similar to other types.
    • IgE Myeloma: IgE is a rare form of multiple myeloma, exhibiting similar symptoms to other types yet being more aggressive and rapidly leading to plasma cell leukemia or other extramedullary spread.

    Causes

    Scientists have yet to determine the precise cause of most cases of multiple myeloma. Nevertheless, they have made strides in understanding how alterations in DNA can lead to the transformation of plasma cells into cancerous cells. DNA is the molecule that holds the instructions for the majority of our cells’ activities.

    • Some DNA contains certain genes that control when our cells grow and divide. These genes that promote cell growth are referred to as oncogenes.
    • Genes that control the rate of cell growth and cause cells to die at the appropriate time are referred to as tumor suppressor genes.

    Mutations in the DNA that activate oncogenes or deactivate tumor suppressor genes can lead to the development of cancer.

    Research has revealed that certain oncogenes, such as MYC, present abnormalities in the initial stages of plasma cell tumors. In contrast, alterations of other oncogenes, such as RAS, are commonly seen in myeloma cells in the bone marrow transplant for multiple myeloma. Moreover, mutations in tumor suppressor genes, for instance, p53, are associated with the metastasis of cancer to other parts of the body.

    Studies also stated that patients with plasma cell tumors have abnormalities in bone marrow cells, which may contribute to excess plasma cell growth. Dendritic cells in the bone marrow secrete IL-6, a hormone that stimulates normal plasma cell growth.

    Diagnosis

    Occasionally, multiple myeloma is identified unexpectedly through a blood test conducted for a different purpose. Alternatively, if your doctor believes you may have multiple myeloma based on your signs and symptoms, they may also order a diagnosis.

    Diagnosing multiple myeloma may involve tests and procedures such as:

    1. Blood tests: A laboratory analysis of your blood can reveal the presence of M proteins produced by myeloma cells. Additionally, beta-2-microglobulin, another abnormal protein produced by myeloma cells, may be detected and provide insight into the aggressiveness of the myeloma. Furthermore, blood tests assessing kidney function, blood cell counts, calcium levels, and uric acid levels can also help to inform the diagnosis.
    2. Urine tests: A urine analysis may reveal the presence of M proteins, also known as Bence Jones proteins.
    3. Tests of your bone marrow: Your doctor may take a sample of bone marrow for laboratory examination by performing a bone marrow aspiration and biopsy, which involves inserting a long needle into a bone. The collected sample will then be tested for the presence of myeloma cells, and specialized tests such as FISH (fluorescence in situ hybridization) can be used to analyze these cells and detect any gene mutations.
    4. Imaging examination: It is possible that imaging tests such as an X-ray, MRI, CT, or positron emission tomography (PET) scan may be recommended to identify any bone issues that may be related to multiple myeloma.
